If you or your loved one have spent time in hospital recovering from injury or illness and need further time in care before returning home, then Aged Care Planning can help find a placement for a short term period of up to 12 weeks. This placement needs to be into a facility or separate ward of a hospital that addresses your care needs whether it be:
- Access to physiotherapy, food and nutrition advice, or podiatry
- Access to a social worker
- Nursing support for clinical care such as wound care
- Personal care
